Our approach<\/h1>\n
EHdK works in partnership with youth care services, mental health and addiction care, and organisations in the social domain to establish integrated family teams: one team around the family, with one plan, one point of contact, and clear communication lines. Setting up these teams involves three steps: <\/p>\n

- Exploration with a working group
Together with the municipality and relevant organisations, we explore what is needed to establish an integrated family team. We determine which partners should be part of the team, how to organise access and coordination, how to handle data safely, what operational and financial agreements are required, and what this approach demands from both professionals and leadership. <\/li>\n - Pilot and evaluation
We start with a pilot team that supports families using the integrated approach. Throughout the pilot, we continuously monitor key questions: Are we able to respond more quickly? Is there a clear, shared analysis and plan? Do families experience the support as truly coordinated? Based on these insights, we refine the working method, agreements, and team structure. <\/li>\n - Structural implementation with ongoing evaluation
After the pilot phase, the team is deployed on a structural basis. EHdK continues to provide support in learning and making adjustments. This ensures that integrated family support becomes not just a temporary project, but a lasting way of working. <\/li>\n<\/ol>\nEHdK supports the entire process: from design to method. We help teams to analyze integrally, prioritize together and ensure that “basis op orde” actually becomes the starting point of integral family assistance. <\/p>\n

\n\n\nCurrent situation<\/h1>\n
Integrated family support is not only necessary \u2014 it\u2019s also feasible. At present, one integrated family team is active in the municipality of Smallingerland. In addition, exploratory work is underway in two other municipalities in Friesland, and funding for family support in the Friesland and Drenthe regions has been secured through the IZA transformation plans. Step by step, we are building an approach that shows integrated family support is possible within the current system \u2014 as long as we organise it together. <\/p>\n<\/div>\n<\/div>\n<\/div>\n
